Forms should be on paper A4 format, with a whiteness 100%, 120g with a color image of the coat of arms, according to the standard documentation rules, fields of at least 20 mm left, 10mm-right,Upper, lower, according to the submitted numbering. Forms should contain the full name of the organization in the state and Russian languages. At the upper part of the form, the state emblem of the Republic of Kazakhstan is printed in color format.
