- 1. The subject of this order are the successive delivery of materials for distribution to the National Oncology Institute of Description of the subject of the contract and the terms of the contract are included inAnnexes:
• Assortment-price specification-Annex 2.1-2.2,
• The contract provisions are planned, which will be entered into the content of the contract-Annex 4.1-4.2 to SWZ
3. The contracting authority allows the possibility of submitting partial offers, and therefore the subject of the contract is divided into 2 separateTasks, including:
Task No. 1 - Successive delivery of materials to distribute
Task No. 2 - Successive delivery of gloves to the chambers
3. The contracting authority provides for the possibility of amending the provisions of the concluded contract (so -called contract changes based on Article 455 (1) point 1 of the Act) in relation to the content of the offer, onthe basis of which the contractor was selected, in accordance with the conditions contained in Annex 4.1-4.2 to SWZ. Amendment to the contract may also take place in the cases referred to in art. 455 para. 1 points 2-4 and para. 2 of the Public Procurement Law.
4. Offer evaluation criteria: price (100%)
